Descemet-stripping automated endothelial keratoplasty (DSAEK) is presently the “gold standard” for the surgical treatment of uncomplicated endothelial failure. As surgical techniques evolve and as results and reproducibility improve, indications are expanding to the point that DSAEK is now indicated for practically all eyes with decompensated endothelium. This video presents the technical modifications that enable the surgeon to perform DSAEK in eyes with challenging features, such as phakic or aphakic eyes, eyes with anterior chamber IOLs, eyes with endothelial dystrophies other than Fuchs, buphthalmic eyes, and eyes with previous glaucoma surgery of all types.
